Initial Consultation
Your initial consultation at the Center for Implant & Aesthetic Dentistry will include a detailed
explanation of your diagnosis and treatment options from our dentists. Our doctors are committed to
helping patients understand their oral health needs. Patients should bring any surgical referrals and
X-rays they have, a list of current medications, and completed dental or health insurance forms to their
initial consultation. Please inform our staff of any existing medical conditions before receiving treatment.
Financing
Fees incurred for dental services are due in full when services are rendered. For your convenience we
accept Cash, Checks, MasterCard, Visa, and Discover. We are also a participating provider for Care Credit®,
our financing partner, which allows you to meet your financial responsibility at the time of service.
Insurance
We accept all major forms of dental insurance. We will attempt to estimate any expenses prior to your
visit to our office. Please be prepared for any deductible or other expenses at the time of service.
If, for any reason, your insurance company does not respond with financial payment within 45 days post
treatment, the balance is due and payable in full immediately by the responsible financial party.

Appointment Policy
We understand that last minute changes in your schedule may be unavoidable and we will try to accommodate
those changes as best we can. Please understand that when we make an appointment we are setting aside
enough time to do our best work and that each appointment is for only one patient. We do not overbook
expecting some of our patients to not show for their appointments. Therefore, a broken appointment
without adequate notice results in wasted time for us, adding to the cost of providing care for all our
patients.
If you have an unavoidable conflict and cannot give 24 hr notice to change the appointment, there will
be no charge, however, before making your next appointment, a $75 fee will be applied. If you come to
your next appointment, the $75 fee will be applied toward the procedure you’re having that day, therefore
there is no extra charge. Alternately, if you have a second, subsequent cancellation for the same
appointment, you will incur a fee of 1/3 the scheduled amount of treatment and lose the initial $75 fee.
A third subsequent cancellation of the same appointment will require prepayment of the entire fee prior
to rescheduling and the fee forfeited if the appointment is not kept.
24-hour notice will give us time, usually, to fill in the appointment and there will be no broken appointment fee.
We will work with you to try to schedule times that are best for you. Patients who habitually reschedule
for inadequate reason will be referred to an office which does not work by appointment or which can
accommodate an unpredictable schedule.
Imaging
At the Center for Implant & Aesthetic Dentistry, we use state-of-the-art technology to view and diagnose
any dental problems that the patient may have. One of these is to use Cone-Beam Volumetric Imaging (CBVI)
provided by 3D Imaging of the CSRA®. This imaging system is comparable to a medical CT Scan allowing
the Dentist to see much more than a traditional x-Ray.
